Introduction

An unscheduled follow up inspection to the Routine Inspection, dated April 12 and April 13, 2022, was conducted.

Current Inspection - Items reviewed comply with the Act, regulations & standards of practice except for those noted on supplementary pages.

Observed Violations
LICENSING: 30030 - RCR s.8(2)(a)(ii) - A licensee must not make any structural change to a community care facility unless the licensee first (a) submits to a medical health officer (ii) a description of how the licensee intends to ensure the health and safety of persons in care while the change is being made.
Observation: Two renovations done in the facility (June 2022 and August 2022) without health and safety plan for persons in care submitted and accepted by Licensing.
Corrective Action(s): A licensee must not make any structural change to a community care facilities unless the licensee first submits to a medical health officer a description of how the licensee intends to ensure the health and safety of persons in care while the change is being made.

PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 31270 - RCR s.21(d) - A licensee must ensure that all furniture and equipment for use by persons in care (d) are maintained in a safe and clean condition.
Observation: Found moderate amount of lint accumulating outside the dryer vent though service was done in May 2022.
Corrective Action(s): Please ensure that all furniture and equipment used by PICs are maintained in a safe and clean condition.

PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 31290 - RCR s.22(1)(b) - A licensee must ensure that all rooms and common areas are (b) maintained in a good state of repair.
Observation: Main staircase ceiling dry wall still shows multiple cracks. Kitchen ceiling dry wall damage caused by water leak has work started, covered with plastic material and taped.
Corrective Action(s): Please ensure that all rooms and common areas are maintained in a good state of repair.

PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 31300 - RCR s.22(1)(c) - A licensee must ensure that all rooms and common areas are (c) maintained in a safe and clean condition.
Observation: The pathway steps to access to the gazebo is still too high for persons in care (PIC), no repair was done. Significant rodent droppings were found in one PIC's room in the basement.
Corrective Action(s): Please ensure that all rooms and common areas are maintained in a safe and clean condition.

STAFFING: 32010 - RCR s.37(1)(a) - A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has obtained all of the following: (a) a criminal record check for the person.
Observation: Two employees have expired criminal record check and no evidence of new CRC application on file.
Corrective Action(s): Please ensure that the licensee does not employ a person in a community care facility unless they have obtained a criminal record check for that person.

One new contravention and four repeat contraventions were identified that would require an action plan.
