Corrective Action(s): - Remove bait boxes from the premises.

Comments

- Required cleaning has been completed.
**Continue to work on regularly maintaining a high level of sanitation throughout the entire premises, especially in hard to reach areas.

- Prep Cooler (across from gas burners): 4C
- Prep cooler door seal has been repaired and can close tightly.
- Cooler inserts cover has been repaired. Duct tape removed, and cover handles are affixed firmly.

- Reviewed recent pest control service report with operator. Pest control services are scheduled monthly. The following actions were taken:
- Potential pest entry points were identified throughout the premises. Operator has scheduled a handyman to block/ cover entry points.
- Operator has contacted landlord to install doorsweeps at all kitchen corridor doors (x3). Doorsweeps should be installed flushed to the ground.
- Additional ketch-all traps have been placed throughout the premises. Operator to request for additional glueboards.

**Continue to monitor for pest activity.
**Report new and/ or increased activity to your pest control company.
**Rodent bait is not permitted inside the food premises.
**Ensure all areas of the premises are accessible for pest control service.
**Regularly maintain a higher level of sanitation throughout the premises.
