Introduction

An unscheduled routine inspection was conducted to assess compliance with the Community Care & Assisted Living Act (CCALA), the Residential Care Regulation (RCR) and the relevant Director of Licensing Standards of Practice (DOLSP). Evidence for this report was based on the Licensing Officer’s observations, review of the facility records and information provided by the facility staff at the time of inspection.

As part of the routine inspection a Facility Risk Assessment Tool is completed and a copy is provided. The Risk Assessment includes non-compliance identified during the routine inspection and a 3 year “historical” review of the facility’s compliance and operation.

Contraventions
Previous Inspection - Contraventions observed on FIR #NBIH-APCRCB have been corrected.
Current Inspection - Items reviewed comply with the Act, regulations & standards of practice except for those noted on supplementary pages.

Observed Violations
PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 31260 - RCR s.21(c) - A licensee must ensure that all furniture and equipment for use by persons in care (c) are maintained in a good state of repair.
Observation: During the routine inspection it was noted that :

- In the Tub room the change table mat had a tear on both sides and both tears were covered with black duct tape

Corrective Action(s): Ensure all furniture and equipment for use by persons in care is maintained in a good state of repair

PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 31290 - RCR s.22(1)(b) - A licensee must ensure that all rooms and common areas are (b) maintained in a good state of repair.
Observation: During the routine inspection it was noted that:
-In 2 bedrooms where the carpet on the wall meets the door frame , the carpet was falling off and was duct taped on
-In 1 bathroom where the carpet in the hall way meets the bathroom door fram the carpet is coming off and is duct taped on.
-The wall in the tub room has paint chips missing and some dents and holes
-The light switch near the "old front door" is broken and missing the switch and there are exposed wires
-The floor in the basement is missing areas of Linoleum and the cement floor is showing through. In areas there is cement missing and there are holes and loose cement
- The outside back patio walkway has broken and cracked concrete

Corrective Action(s): Ensure all rooms and common areas are maintained in a good state of repair

PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 31300 - RCR s.22(1)(c) - A licensee must ensure that all rooms and common areas are (c) maintained in a safe and clean condition.
Observation: During the routine inspection it was noted that the Mechanical room had
-old equipment stored
-dust and dirt on the floors, walls and window sills
-water/dampness on the floor
-old chemicals stored on the floor
-a noticeable musty/mildew smell
Corrective Action(s): ensure all rooms and common areas are maintained in a safe and clean condition

PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 31860 - RCR s.69(3)(a) - A licensee must ensure that (a) all medications in the community care facility are safely and securely stored.
Observation: It was noted during inspection that one of the locks on the medication cupboard is broken
Corrective Action(s): ensure that all medications are safely and securely stored
