130 - Products / instruments not protected from contamination Observation: * Some dirty/used hair implements were being stored with clean ones-- which is unsanitary practise and this can lead to contamination of cleaned hair implements. All dirty implements must be properly cleaned and sanitized prior to storage Corrective Action(s): * Ensure dirty implements not stored with cleaned/sanitized ones. All cleaned implements should be stored properly in closed containers to reduce risk of contamination Violation Score: 9 330 - Products / instruments not protected from contamination Observation: * Some clean esthetic implements were found being stored with dirty ones and this can lead to conatmination of cleaned implements. Corrective Action(s): * Ensure all dirty esthetic implements are properly washed/sanitized prior to storage to reduce risk of contamination of cleaned implements Violation Score: 9 333 - Equipment / instruments / products not of suitable material and/or not maintained in good repair Observation: * Several hand washing stations did not have adequate soap in dispensers. This has been problem in this premises in the past and must be addressed and maintained regularly for proper means of hand sanitation Corrective Action(s): * Ensure ALL hand sinks have adequate soap and paper towels available at ALL times during operation and maintain Violation Score: 12

* Services: hair/waxing/threading/nails/laser * Several hand washing stations did not have soap available for proper hand washing/sanitation * Note: operator to reorganize drawers and implements to ensure all areas maintained in sanitary condition; cleaned tools/implements to be stored properly to protect them from contamination(eg. in closed containers, etc) * Note: Barbicide container had visible build up of hair particulate at bottom of jar; operator to clean and refill with new barbicide and to clean all hair implements prior to placing them into barbicide disinfectant * Follow up inspection: March 29th, 2018
