PHYSICAL FACILITY, EQUIPMENT AND FURNISHINGS: 31100 - RCR s.17 - A licensee must ensure that water accessible to a person in care, from any source, is not heated to more than 49° Celsius.
Observation: One bathroom temperature was noted at 51.4° Celsius.
Corrective Action(s): Ensure that water accessible to PICs from any source is heated no more that 49° Celsius.

POLICIES AND PROCEDURES: 33170 - RCR s.74(1)(b)(i) - Subject to subsection (2), a licensee may restrain a person in care (b) if there is agreement to the use of a restraint given in writing by both (i) the person in care, the parent or representative of the person in care or the relative who is closest to and actively involved in the life of the person in care.
Observation: Restraints are used for preventative measures to ensure PIC’s safety. No PIC consents were on file.

Observation: No Physician or Nurse practitioner approval is in place for the restraints.

NUTRITION AND FOOD SERVICES: 37090 - RCR s.62(2)(d) - A licensee must ensure that each menu provides (d) for substitutions to be made that are from the same food group and have a similar nutritional value.
Observation: Review of posted menu substitution form found 4 days of lunch menu having substituted with different food groups and 2 days of dinner menu substitution listed as "Leftovers".
Corrective Action(s): Ensure that all menu substitution is documented clearly identifying the similar food groups.

RECORDS AND REPORTING: 39330 - RCR s.83(4)(c) - Subject to subsection (5), a licensee, other than a licensee who provides a type of care described as Hospice, must (c) record the weight in the nutrition plan of the person in care.
Observation: Review of online monthly records found that all PICs weight for October was not documented and 1 PIC weight was missing for September on the PICs files.
Corrective Action(s): Ensure that all PICs are weighed monthly and weight documented appropriately.
