Observation:
- Observed dead & live cockroaches on sticky traps inside kitchen.
- Mouse droppings observed throughout - mainly on the bottom shelving racks in the dry storage rooms (ground floor and 2nd floor)
Corrective Action(s):
- Monthly pest control in place by third party pest control company. Increase the frequency to biweekly (2X a month).
- Ensure a treatment service is done for cochroach control. Service report will be reviewed.
- Remove all items on the shelves to facilitate proper cleaning. Sanitize any areas with rodent droppings.

Observation:
- Dust build up observed on ceilings around the vents (inside the kitchen).
- Shelving units with counter tops observed with food debris.
- Debris observed on the interior surface of the ice machine.
Corrective Action(s):
- Overall deep cleaning required in the kitchen. Ensure all surfaces (including the floors) are cleaned.
- Clean & sanitize the interior of the ice machine after turning the machine off & emptying all ice out.
