Introduction

An unscheduled routine inspection was conducted to assess compliance with the Community Care and Assisted Living Act (CCALA), the Residential Care Regulations (RCR) and the relevant Director of Licensing Standards of Practice (DLSP). Evidence for this report was based on the licensing officer’s observations, review of the facility records, and information provided by the facility staff at the time of inspection.
The following areas were reviewed:

· Care and Supervision · Records and Reporting

As part of this Routine Inspection a Facility Risk Assessment Tool is completed and a copy is provided. The Risk Assessment includes non-compliance identified during the routine inspection, and a 3 year ‘historical’ review of the facility’s compliance and operation.

Observed Violations
CARE AND/OR SUPERVISION: 34630 - RCR s.81(3)(b) - A care plan must include all of the following: (b) an oral health care plan.
Observation: Of 4 care plans reviewed, 3 did not include an oral health care plan.
Corrective Action(s): Ensure that all care plans include an oral health care plan.

CARE AND/OR SUPERVISION: 34640 - RCR s.81(3)(c)(i) - A care plan must include all of the following: (c) a nutrition plan that (i) assesses a person in care's nutrition status.
Observation: Of 4 care plans reviewed, 3 did not include a nutrition plan indicating the PIC's nutrition status was assessed.
Corrective Action(s): Ensure that all care plans include a nutrition plan.

CARE AND/OR SUPERVISION: 34760 - RCR s.81(4)(b)(ii) - A licensee must ensure that (b) each care plan is reviewed and, if necessary, modified (ii) if there is no substantial change in the circumstances of the person in care, at least once each year to ensure it continues to meet the needs and preferences, and is compatible with the abilities, of the person in care who is the subject of the care plan.
Observation: Of 4 care plans reviewed, 3 did not appeared to have been reviewed at least once per year. One care plan had been recently developed as the PIC was recently admitted to the facility.
Corrective Action(s): Ensure that all care plans are reviewed annually to ensure it continues to meet the needs of the PIC.

RECORDS AND REPORTING: 39090 - RCR s.77(2)(c) - Subject to subsection (3), if a person in care is involved in a reportable incident, the licensee must immediately notify (c) a medical health officer, in the form and in the manner required by the medical health officer.
Observation: Upon review of facility records, it was determined that an incident occurred, requiring the home to be evacuated for approximately 6 hours. This would be deemed a reportable incident as defined in Schedule D of the Residential Care Regulation.
Corrective Action(s): Ensure that the medical health officer is notify when a PIC is involved in a reportable incident.

RECORDS AND REPORTING: 39470 - RCR s.87(c) - A licensee must keep a record of the following matters respecting food services: (c) the results of monitoring, by the licensee, of food services and nutrition care.
Observation: Upon inspection it was determined that a menu audit has not been completed. Of concern is that the menu does not meet the nutritional requirements of this facility's PICs. It was also noted that the last documented review of the menu was in 2000.
Corrective Action(s): Ensure that records are maintained concerning monitoring of food services and nutritional care.

Comments

Areas unable to be reviewed due to the Manager not being on-site during the inspection include: staff records, PIC financial records, and the most recent medication, safety and advisory committee meeting minutes (MSAC). These areas will be reviewed at a later date to ensure compliance.
