317   Inadequate or improper construction of water works
Observation: Changes to system (sand filters and UV). Previous site map/treatment system schematic insufficient. Provide a: - Detailed, labelled site map of the property. Show source, septic, distances, water lines, connections, structures, etc. - Detailed treatment schematic of the current system, going from source to the end. Include all specifications and specifics on the makes/models. Provide clear photos to support.
319   Inadequate Chemical Analysis Data
Observation: Last chemical/physical analysis on file from 2014. These need to be done every 5 years. Operator stated that a recent full analysis was done for another program (GAP?). Submit a copy to this office to review. If insufficient, then a full chemical/physical analysis of the raw, untreated water needs to be completed.
321   Inadequate Treatment
Observation: Facility on a Boil Water Notification (2017). Current system design does not meet Surface Water Treatment objectives. Upgrades will be required. Baseline information needs to be updated.

Provide the complete site map and treatment schematic by January 14, 2022. If the chemical/physical analysis for the GAP program is insufficient, complete a full analysis of the raw water by January 14, 2022.
