Report of Findings
On Sept 8, 2021 Licensing received a complaint alleging concerns regarding policies and procedures.

The allegations included the following details:
- a staff memeber does not have the appropriate training to administer medication

All allegations were thoroughly investigated. Evidence was received and reviewed from interviews, facility documents, and a complaint inspection at the site.

Based on the evidence reviewed, the following contravention was determined:
